How it works

Who needs this training

Staff who investigate, or contribute to investigations of, incidents and complaints:

We recommend at least Level 1 literacy and numeracy.

What the law says

In England, CQC's Regulation 17 (good governance) expects providers to assess, monitor and improve the quality and safety of their services, and Regulation 20 (duty of candour) requires them to be open with people when things go wrong. Investigating incidents well — finding the contributing factors and acting on them — is how a service shows it learns.

For NHS-funded care in England, NHS England's Patient Safety Incident Response Framework (PSIRF) has replaced the Serious Incident Framework. It favours system-based learning responses over traditional root cause analysis. Many independent and social care providers still use RCA, and its core skills carry across.

Logistics

  • Room: horseshoe or classroom seating — what matters is clear floor space — with a projector screen or blank wall
  • The trainer brings: projector, laptop, flipchart and all general training equipment

  • Access: the trainer needs the room 45 minutes before the start

Frequently asked questions

Is RCA still used, given PSIRF?

NHS-funded services in England now use PSIRF learning responses rather than traditional root cause analysis. Many independent and social care providers still use RCA, and the core skills — gathering facts, building a timeline and looking at systems rather than individuals — carry across.

Is this PSIRF training?

No. It teaches root cause analysis. If your service follows PSIRF, tell us when you enquire so we can confirm the course fits what you need.

Which option should we book?

Can we use one of our own incidents?

Yes, anonymised. Send it in advance and the trainer will build the case study around it.
